FDAに承認されている薬を、指示通りの用法に従わないで使用すること。

otherwiseの使い方。

In this case, more information should reduce any confusion that might otherwise arise. この場合、情報が多い方が余計な混乱を避けられる。

one or both 片方またはどちらも

"xxx shall meet one or both (a) and/or (b)" 「(a)と(b)のどちらか、または両方を満たさなければならない」と書かれると、meet (a) or (b) だけで良いのではないかと思いがち。

ところがmeet (a) or (b)としか書かないと「(a)と(b)の両方を満たしてしまったら？」と聞かれることになる。
